Official AKC Hunt Test or Field Trial Entry Form (Word .doc)

 

Unlike other PDF or Locked File documents, here is a Word document (.doc) that will serve as any Hunt Test or Field Trial entry. All you need to do is:

  1. Upload the file (click on the link below)
  2. Save to your hard drive
  3. Open the file in Word
  4. Add your dogs and personal information, i.e. Breed, AKC #, Sex, D.O.B., Full Name, Call Name, Sire, Dam, Breeder, Owner, Address information, Handler, Telephone Number and Email Address.
  5. Save the file again as a template and now you can fill in the information for any trial or hunt test and send it directly to the Club holding the event.

 

Please note: the "Oval" in the Event Type box moves. Just right click on it once and drag it side-to-side.
